Heywood Rural Health (HRH) is pleased to announce that it will become one of three new providers of Commonwealth Home Support Program (CHSP) services in Glenelg Shire Council.
HRH together with Portland District Health and Western District Health Services (on behalf of Casterton Memorial Hospital) will take over a range of CHSP services from Glenelg Shire Council effective 1 October 2023.
CHSP services assist people over the age of 65 to maintain independence in their home through support with daily living tasks, home modifications, home maintenance services and social activities.
Services to be transitioned to HRH include Domestic Assistance, Flexible Respite, Personal Care, Social Support, Meals on Wheels, Home Modifications and Home Maintenance (e.g. lawn mowing). This will include delivering services to over 50 new clients within the Heywood region.
The transition also includes providing Home Maintenance (e.g. lawn mowing) for Heywood clients, including additional clients within the Portland catchment.
HRH team are working diligently to ensure a seamless transition of services occurs by October 1st 2023.
